Alternatively, we can email you a copy of the correct form to complete and send back. We can provide the forms in MS Word or PDF format.

Outdoor Activity Booking Form
Once you have made an outdoor activity booking with us, we will need to gather some details from each participant ahead of the day using the Individual Booking & Equipment Form. We will require some basic contact details, a nominated emergency contact, basic medical information, and ask that you agree to a participation statement. Additionally, we may need some measurements from you to provide the correct size clothing and equipment for your activity.
